I have a family member who is going to park and live in their 5th wheel at my house for several months. The 5th wheel does not have a black water tank (not sure why). We are planning on hooking the sewer hose directly to the cleanout port of the house, but the inlet is level with the RV's outlet. I don't think we will get proper flow with this setup. Raising the 5th wheel is not an option due to clearance. Any ideas how to make this work?

The clean out connects to the sewer line beneath the ground. Sounds like you need to create a sewer line riser away from your house. If you are not able to do that job yourself, I suggest you contact a plumber. He'll (she?) will let you know if it's possible for your situation. But one rule always stands...the sewage will always run down hill. Good luck... OR THAT ABOVE ? (added)
